The characteristics of our broadly cross-reactive H5 MAbs strongly suggest that they may recognize 1 or more highly conserved antigenic determinants in the HA protein of H5N1 virus. To understand the molecular basis of how 13D4 recognizes proteins in antigenically diverse H5N1 viruses, we mapped 13D4 MAb binding sites in the HA protein. The following 3 escape mutants were generated: R189M, derived from A/IDN/5/2005, and K152E and N182K, derived from A/VNM/1194/2004. Hemagglutination inhibition assays with 13D4 and other cross-reactive MAbs showed that these escape mutants still retained hemagglutination inhibition, albeit at a lower level than their parental viruses (table 3). Because A/VNM/1194/2004 and A/IDN/5/2005 belong to the antigenically distinct H5N1 virus clades 1 and 2.1, respectively, they may represent different natural immune escape mutants. Amino acid position 189, which corresponds to antigenic site B of H3N2, may represent an immunodominant epitope, and mutations at this position may thus be subject to strong positive selection [36]. As demonstrated previously [15], A/VNM/1194/2004 contains a mutation in position 189, R189K. Its continued recognition by 13D4, together with the generation of alternate-site escape mutants observed here, suggests that 13D4 binds to multiple sites in HA, possibly preventing immune escape associated with single-site mutations.

No, the R193M change signals immunological escape allowing the H5N1 to spread in humans (and form clusters like the one seen in Bali).
