Summary:
In weeks 23-24/2010 17 samples have been sent to the laboratories of the sentinel system of which three have been positive for seasonal influenza virus B (18%) all notified by the network of the Canary Islands in a population under age 15.

A total of four detections have been reported by sentinel and non sentinel, 25%
correspond to AnH1N1 and 75% to influenza virus B.

It's been updated information on severe clusters of AnH1N1 in Galicia and the
outbreak of influenza B in the population under 15 years in the Canaries.

It has also been reported a detection of pandemic virus
from the Laboratory of Galicia.

To date, we have identified 18 viruses with the D222G mutation in the HA1 region
distributed hemagglutinin: Andalusia (3), Castilla La Mancha (4), Catalonia (3),
Galicia
(5)
and the Basque Country (3). They have been detected in critically ill patients. We have also identified 99 strains with the D222E mutation in patients in serious and milder condition coming from different regions.

To date 12 have been identified with the mutation H275Y virus spread over
Andalusia (2), Baleares (1), Catalonia (1), Galicia (3), Madrid (3), Navarra (1) Basque Country (1).
Epidemiological data collected show that all patients were treated
oseltamivir prior to the detection of the resistant strains.
All of the M2 protein sequences analyzed from week 40 / 2009 (105) show
S31N mutation conferring resistance to the antiviral adamantane family

Other information
From the Public Health Department of Galicia have been updated two new
severe cases of pandemic virus confirmed in Ourense Hospital Complex

(CHOU), so far, were reported six severe cases of confirmed
AnH1N1 between weeks 19-22/2010 (9 to 26 May). The new case
reported in week 23/2010 is a woman of 73 years, She has asthma, with a history
2009-10 seasonal vaccination and vaccine
pandemic immunization status unknown
; not need to enter the ICU and was discharged. The second case
reported in week 24/2010 is a woman of 77 years, asthma, with vaccine
Seasonal administered and no history of pandemic vaccination. She is
in stable condition without needing the ICU. None of these cases has
connection with the previously reported cases...

children predominantly "We think that the etiology of infection is a
virus".


According to the Department of Health, no case has required
hospitalization since the disease emerged, which begins with a dry cough
followed by persistent fever and presents a bening course in all
those affected, who have recovered with broad spectrum antibiotics,
antipyretics and in some cases, bronchodilators.


Before starting to report this outbreak to the epidemiological service on 3 August, the department has maintained contact with clinicians in the affected area and has coordinated with the emergency medical services and the Manacor Hospital, Hospital Universitario Son Dureta as with various health private centers.

The Balearic Executive also remarked that at present there is no
signs of any environmental source or any place where there's a higher risk, so the only measures to take are the usual hygiene to prevent
transmitted through droplets or direct contact.

Virological situation:

In the week 42/2010, 74 samples have been sent to laboratories by sentinel system of which two were positive for influenza virus that is a viral detection rate of 2.7%. One of them has been a seasonal flu virus A/H3 detected in a man, 58, in Catalonia with history of influenza vaccination. The second is a virus detection influenza B reported through the network of Asturias in a two year old with no history of vaccination.

In week 42/2010 has reported two sentinel detection of influenza virus AH3 and B
no virus AnH1N1 (2). Since the beginning of the 2010-2011 season five viruses have been reported positive for influenza (60% sentinel and 40% non-sentinel): 1 AH3 and 4B.

In week 42/2010, The National Microbiology Center has not updated information on genetic characterization, and antiviral resistance mutations in strains of flu.



Comments:
- In week 42/2010 the overall incidence rate of influenza is 18.23 cases per 100,000 population and remains well below the baseline established for the 2010-2011 season.

- The territory under vigilance indicates a low level of influenza intensity and
no spread of the disease.

Epidemiological situation

In week 43/2010, at the end of current reporting period data of influenza has been received from all sentinel groups, the SVGE integrated networks. Overall, levels of influenza continue to be low with no geographic spread of the disease and a stable trend compared to previous weeks.

Overall incidence rate of influenza in week 43/2010 is 16.49 cases per 100,000 people, remaining well below the baseline established for this season. In the ILI distribution by age groups the highest incidence rates are recorded in people under five years.

Virological situation

In the week 43/2010, 71 samples were sent to the laboratories of the system of which three tested positive for influenza virus which is a viral detection rate of 4.2%. One of them is a pandemic virus, the first of the 2010-2011 season detected in a man of 25 years in Navarra, another virus of seasonal influenza A not subtype correspond to a man of 44 years in Baleares, influenza C was reported by the network of Catalonia in a girl of one year, none of these cases had a history of influenza vaccination.

The table below shows the number of sentinel and non-sentinel virus detections reported since the beginning of the 2010-2011 season.
In week 43/2010, three sentinel detection of influenza virus were reported: 1A is not subtyped, 1 AnH1N1 and 1 C. From the beginning of the 2010-2011 season has been reported eight influenza virus (75% sentinel and 25% non-sentinel): 1 A not subtyped, 1 AnH1N1, 1 AH3, 4B and 1C.

In the SVGE we have taken since week 20/2009 the nomenclature of pandemic influenza virus A (H1N1) 2009 as AnH1N1 for technical modifications to the notice through the website of SVGE [url]vgripe.isciii.es/gripe[/url].

So far, there has been a mixed flow pattern of influenza virus AH3N2, AnH1N1, B and C.
From week 40/2010, five detections of RSV have been reported from the Laboratory of Hospital N ª Sr ª de Covadonga (Hospital Central de Asturias) in Oviedo, from the Laboratory of the Hospital of Son Dureta Palma de Mallorca (Baleares) and the Laboratory Hospital Virgen de Arrixaca in Murcia.

-61 sentinel samples have been sent to the laboratories of the system of which two have been positive for influenza virus (3.3%): 1 AnH1N1 and 1 B.

On November 2, a severe case was reported to be hospitalized and confirmed with AnH1N1.

It is worth noting that a growing trend for three consecutive weeks in the incidence rate of influenza in the 0-4 age group in Asturias and in the 15-64 years in Navarra. From the beginning of the 2010-2011 season Asturias has only recorded flu B cases, usually associated with a greater involvement of school population while Navarre has only seen the cirucalation of AnH1N1 and AH3N2 of the sub-typed flu viruses.

Laboratory Results

In the week 44/2010, 61 sentinel samples have been sent to the laboratories of the system of which two were positive for influenza virus representing a viral detection rate of 3.3% (Figure 3). One of them has been an AnH1N1 detected in a child of five years in Navarra, and an influenza B virus reported by the network of La Rioja in a child of years, none of these cases had a history of influenza vaccination. This week has been updated a total of six detections of influenza virus of the week 43/2010: Two AH3N2 from Madrid and Navarra, and four B in Asturias notified in the school population.

In addition, the laboratory of the Basque Country has reported a non-sentinel detection of influenza B in a man of 81 years with unknown vaccination status.

Table 1 shows the number of sentinel and non-sentinel detections reported since the beginning of the 2010-2011 season. From the beggining of the 2010/ 2011 season 19 influenza viruses have been reported (60% sentinel and 40% non-sentinel) of which 32% have been of type A (1 A not subtyped, 2 AnH1N1, 2 AH3, 1 AH3N2), 63% type B and 5.3% of type C 89% of all detections reported from week 40/2010 are from areas located in the north of Spain.

Surveillance for severe influenza cases confirmed and hospitalized

In week 44/2010, a severe case was confirmed with AnH1N1 and hospitalized.

This is a 49 year old man who was admitted to the hospital on November 2 (week 44/2010). The patient has chronic cardiovascular disease and was admitted to the ICU with a secondary bacterial pneumonia. He has been treated with oseltamivir. So far, It's not known his history of influenza vaccination with monovalent vaccine against AnH1N1 and the flu vaccine for this season 2010-2011.

(EFE) .- Two people have died because of the flu in Spain so far this year, one with multiple risk factors while the other had none.

This follows from the newsletter Surveillance System Influenza in Spain, according to data collected between 28 November and 4 December. One of the dead had multiple risk factors and was killed in the middle of last month, while the other, a boy of 42 years, had no health problems.

This patient entered the ICU on November 29, he developed several complications and died on 1 December, according to information from the surveillance network. Both agreed to not being vaccinated against influenza and that the initiation of treatment with oseltamivir was more than 48 hours after onset of symptoms.

Since the beginning of the 2010-2011 season have been reported five confirmed cases of hospitalized severe flu, all men, one in the range of 15-44 years and four in the 45-64 years age.

Four of them have been confirmed H1N1 virus, and the fifth, AH3N2 influenza virus. All suffered pneumonia and only one patient did not require ICU admission. None of the reported cases were immunized with influenza vaccine this season.

The overall incidence rate of influenza during the study period is 22.71 cases per 100,000, still below the baseline established for the 2010-2011 season, while it exceeded Asturias and Extremadura for the first time this week.
Overall, slightly intensifies viral circulation, with a mixed pattern of influenza viruses but with a predominance of H1N1 pandemic virus.

Indeed, health officials last week expressed concern about the small number of people who have been immunized against influenza in previous years, at this stage of the campaign and on the eve of the highest peak of infection.

The secretary general of the Ministry of Health, José Martínez Olmos, insisted in a press conference on the need to immunize the entire population at risk and confessed that in some communities, the vaccination rate is at this time to 60 % lower than in previous years.

The highest peak of the flu are expected in the months from January to March, where a decrease in the number of people vaccinated may be crucial to increase the infections or there is a greater impact on mortality from this virus.

Olmos stressed that to the extent that the influenza vaccination coverage are higher, the virus that spreads easily, is found more barriers to be passed from person to person.
He therefore recommended target groups-over 65, people with chronic illnesses, pregnant women and health professionals, among other groups, who come to get vaccinated.
