The coronavirus outbreak in eastern Saudi Arabia has gotten bigger.

The country's deputy health minister says an additional three infections have been confirmed, bringing the cluster to 13 cases.

Of those cases, seven have died.

Dr. Ziad Memish confirmed the three additional cases in an email to The Canadian Press and says the cases are linked through a connection to health care, but does not elaborate on what that link is.

Experts have been on the lookout for infections spreading in hospitals because health-care facilities played a key role in the SARS outbreak.

This new coronavirus is a member of the same virus family as SARS and causes a similarly severe disease.

The start of this outbreak was first reported to the World Health Organization late last Wednesday. At that time, there were seven cases, five of whom had already died.

On Friday, Saudi authorities informed the WHO that three more cases had been confirmed. One was a family member of one of the original seven.

Cases within a family or household, or among people who share a workplace or some other common gathering point would raise alarm bells as it this type of infection pattern could be a signal that the virus is acquiring the ability to spread from person-to-person.

There have been reports that this cluster of cases, in a city called al-Ahsa, are all linked in some way to a hospital there. A report Memish submitted on Friday to the Internet-based disease alert system, ProMED-Mail, suggested all the patients had underlying illnesses.

In fact, the first seven cases were all listed as having "multiple comorbidities.'' A comorbidity is medical-speak for a disease or condition.

The new cases bring the global confirmed count to 30 cases. Of those, 18 people have died.

The first known cases of the new infection occurred in April 2012, in a cluster of 11 illnesses in a hospital in Jordan. At the time, a reason for the outbreak wasn't found.

But months later stored samples from two of those cases were tested and found to be positive for the new virus. The other nine people are suspected to have been infected, but it's not clear at this point if they will ever be confirmed.

The coronavirus was first spotted when a Saudi man died of a mysterious and severe pneumonia last June. When the cause of his illness could not be detected, an infectious diseases specialist sent a sample to Erasmus Medical Centre in Rotterdam, the Netherlands, where virologists determined a new coronavirus was behind the infection.

To date, Saudi Arabia has recorded the highest number of cases. Qatar, the United Arab Emirates and Britain have also had cases. Britain has seen the first confirmed cases of limited person-to-person spread, in a family where three members fell ill and two died. It is believed, though, that the first case in that cluster contracted the virus while travelling in Saudi Arabia.

Dear colleagues,

As a follow up to the most recently reported 10 case cluster of nCOV in Alhasa in the Eastern Province of KSA. Our investigation is still ongoing and we picked up 3 more cases:

Case 11: 62 year old female with multiple comorbidities. Start of symptoms [19 Apr 2013] and deceased [3 May 2013]
Case 12: 71 year old male with multiple comorbidities. Start of symptoms [15 Apr 2013], deceased [3 May 2013]
Case 13: 58 year old female with comorbidities. Start of symptoms [1 May 2013] and currently ventilated in critical but stable condition.

So far there is no apparent community transmission and transmission seem linked to one HCF [health care facility].

We will keep you posted on progress

[ProMED-mail would like to thank Dr Memish for sharing this information on the 3 new cases associated with the cluster of severe acute respiratory illness (SARI) cases in Alhasa, Eastern Province.

As enumerated in the above report, there are now 13 cases associated with this cluster, 7 of whom have died (54 per cent). As with the 10 previously reported cases, all 3 of these have existing comorbidities. The statement from the above report -- "So far there is no apparent community transmission and transmission seem linked to one HCF [health care facility]" supports a conclusion that this outbreak is a nosocomial outbreak, and the reason that all confirmed cases to date have had existing comorbidities is that they were presumably infected with the nCoV while hospitalized in this HCF for other pre-existing illnesses.

The dates of onset of the 13 cases in the cluster in Alhasa are:
1 case on 14 Apr 2017;
1 case on 15 Apr 2013;
2 cases on 17 Apr 2017,
1 case on 19 Apr 2013;
4 cases on 22 Apr 2013;
1 case on 27 Apr 2013;
1 case on 28 Apr 2013;
1 case on 30 Apr 2013;
1 case on 1 May 2013.

As for route of transmission within the hospital, we await findings of epidemiologic investigations currently under way.

As of today, 5 May 2013, the breakdown of total confirmed cases of SARI associated with infection with the nCoV (and deaths) by country of report is:
- Saudi Arabia: 22 (13 deaths)
- Jordan: 2 (2 deaths)
- UK: 4 (one patient from Qatar -- under treatment, 3 patients from UK -- one with history of travel to Saudi Arabia and Pakistan prior to illness; one recovered, 2 deaths)
- Germany: 2 (one patient from Qatar -- discharged, one patient from the UAE -- one death).

Including the 3 newly confirmed cases, there are now 30 confirmed cases with 18 deaths (60 per cent CFR).

The Ministry of Health in Saudi Arabia has informed WHO of an additional three laboratory confirmed cases of infection with the novel coronavirus (nCoV). Two of the patients died on 3 May 2013 and one is currently in critical condition.

The government is conducting ongoing investigation into this outbreak which is linked to one health care facility. Since the beginning of May 2013, a total of 13 patients have been reported from this outbreak, of which seven have died. Of the 13 patients, 10 are men and three women. The age range of the patients are from 24 to 94 years old.

From September 2012 to date, WHO has been informed of a global total of 30 laboratory confirmed cases of human infection with nCoV, including 18 deaths.

Based on the current situation and available information, WHO encourages all Member States (MS) to continue their surveillance for severe acute respiratory infections (SARI) and to carefully review any unusual patterns. WHO is currently working with international experts and countries where cases have been reported to assess the situation and review recommendations for surveillance and monitoring.

All MS are reminded to promptly assess and notify WHO of any new case of infection with nCoV, along with information about potential exposures that may have resulted in infection and a description of the clinical course.

WHO does not advise special screening at points of entry with regard to this event nor does it recommend that any travel or trade restrictions be applied.

WHO continues to closely monitor the situation.
