The above March 8 translation describes the familial cluster who entered the Fifth Peoples hospital between February 14th to 24th with severe pneumonia. By the time of the above media report one son was dead, as was the father. The other son recovered. The father was the first confirmed H7N9 case and the sequences. A/Shanghai/2013, was released when the first three confirmed cases were announced three weeks later on March 30.
At that time WHO should have been well aware of this H2H cluster.


To play devil's advocate (or a Chinese health official), non H2H family clusters are possible when they have the same poultry source. However, the onset dates would need to be the same.

Here, we have a family with different sequential onset dates? Therefore H2H transmission?

There is a big gap in disease onset dates (the father did not develop symptoms until his son had been hospitalized for 5 days). Moreover, the father did not leave his house in the two weeks prior to disease onset. The three family members were hospitalized between March 14 and 24, The first would have been one or both sons, while the father was hospitalized a week after son(s). The younger son died in February, the father died on March 4.

Here is a quick summary. WHO and China did all that they could to "hide" this cluster, which involved H2H transmission in February. The cases were not announced until the end of March, even though the media described the cluster on March 7. Bloggers who said the deaths ere due to flu were arrested and fined for spreading false rumors. After the announcement, media reports noted that there were SEVERAL PCR confirmed cases at the Fifth Peoples Hospital, including additional deaths and discharges. One was the second confirmed death, whose family threatened to sue the hospital because they said the cluster family infected him (27M). However, the sequences didn't match because the second case was infected by OTHER cases at the hospital (representing more H2H). The hospital compensated the family of the second confirmed case (because an investigation would conclude he WAS infected at the hospital and there were multiple UNREPORTED cases). Two of these unreported cases were also disclosed today (there were four disclosures at the Fifth Peoples hospital). The two confirmed serologically were the sons, while the two PCR confirmed were two of the UNREPORTED cases.

Thus, China has now acknowledged SIX confirmed cases at the hospital., including 4 deaths, but these were acknowledged a month after the fact and involve multiple examples of H2H transmission in the house of the familial cluster, as well as the hospital.

I'm afraid I have no confidence in the info we are receiving from there.

Makes you also wonder what other H2H clusters there are that we don't know about.

Holi canoli. With the above scenario, the cat is WAY out of the bag and has been for more than a month. Who do you think is responsible for this obfuscation and what do they hope to accomplish? From my perspective, there is no stopping this now. Why beat around the bush about it?

The above media report from April 1 describe additional PVR confirmed cases which appear to be at least one more than reported today. Thus, todays report brings the number of confirmed cases at the hospital to six, while the media report claims at least 7. The second confirmed case was also at the Fifth People's hospital and was likely infected by another patient because the father's sequence (A/Shanghai/1/2013 did not match the second case, A/Shanghai/2/2913.
The family of the second case was compensated fir the death of the second case, suggesting there was evidence that he was infected at the hospital, representing more H2H spread.

The Ministry of Agriculture indicated 47,801 birds were tested to get 39 H7N9 positives. H7N9 in poultry is rare and the avian sequences lack PB2 E627K, which is in all four human sequences.

ALL of the current H7N9 cases in China are due to H2H.

Human infection with influenza A(H7N9) virus in China – update

3 April 2013 - On 3 April 2013, the China Health and Family Planning Commission notified WHO of an additional four cases of human infection with influenza A(H7N9). The four patients are from Jiangsu province in eastern China. There is no link between the cases.

The patients include a 45-year-old woman with illness onset on 19 March 2013; a 48-year-old woman with illness onset on 19 March 2013; an 83-year-old man with illness onset on 20 March 2013; and a 32-year-old woman with illness onset on 21 March 2013. All of these patients are in a critical condition.

To date, the total number of confirmed cases of human infection with influenza A(H7N9) virus in China is seven. Three confirmed cases were reported earlier from Shanghai and Anhui provinces, including two deaths.

More than 160 close contacts of these four cases in Jiangsu province are being closely monitored. Thus far, none of them have developed any symptoms of illness. Retrospective investigation is ongoing into two contacts of one of the cases reported earlier from Shanghai. Both of these contacts developed symptoms of illness; one died and the other recovered. No laboratory confirmation is available for these two contacts.
